**Call your healthcare provider immediately if you experience symptoms of lactic acidosis, such as:**
- Extreme weakness or fatigue
- Unusual muscle pain
- Difficulty breathing
- Unexplained sleepiness or prolonged sleep
- Stomach or intestinal issues, including nausea, vomiting, or diarrhea
- Coldness in the arms and legs
- Dizziness or lightheadedness
- Slow or irregular heartbeat

**You may be at higher risk for lactic acidosis if you:**
- Have kidney issues (do not take METFORMIN XR if your kidneys are not functioning properly).
- Have liver problems.
- Suffer from congestive heart failure requiring medication.
- Frequently consume large amounts of alcohol or binge drink.
- Become dehydrated (due to fever, vomiting, diarrhea, or excessive sweating).
- Undergo certain x-ray tests involving injectable dyes or contrast agents.
- Have surgery.
- Experience a heart attack, severe infection, or stroke.
- Are over 80 years old and have not had kidney function tested.

**Do not take METFORMIN XR if you:**
- Have kidney problems or abnormal creatinine levels (not to exceed 500 mg daily).
- Are allergic to metformin or any ingredients in METFORMIN XR (see the medication guide for the full list).
- Are scheduled for an x-ray involving dye or contrast agents, or if you will have surgery and be unable to eat or drink. You may need to temporarily stop taking METFORMIN XR; consult your healthcare provider about when to resume it.

**What to Discuss with Your Healthcare Provider:**

Before starting METFORMIN XR, inform your healthcare provider if you:
- Have type 1 diabetes (METFORMIN XR is not for type 1 diabetes).
- Have a history or risk of diabetic ketoacidosis (high ketone levels in the blood or urine).
- Have kidney or liver problems.
- Have heart issues, including congestive heart failure.
- Are over 80 years old (ensure kidney function is normal).
- Drink alcohol frequently or in large amounts.
- Are taking insulin or any other medications.
- Have any other medical conditions.
- Are pregnant or planning to become pregnant. Discuss blood sugar management during pregnancy with your healthcare provider.
- Are breastfeeding or plan to breastfeed. Consult your healthcare provider about breastfeeding while taking METFORMIN XR.

**Keep your healthcare provider informed about all medications you take**, including prescription and over-the-counter drugs, vitamins, and herbal supplements. Be aware that METFORMIN XR may interact with other medications.

**Common side effects** of METFORMIN XR include diarrhea, nausea, and upset stomach, which often subside after continued use. Taking the medication with meals can help mitigate these effects. Notify your healthcare provider if side effects are severe, persist for more than a few weeks, return after subsiding, or occur later in treatment; you may require a dosage adjustment or temporary cessation.

Some individuals (about 3 in 100) may experience a temporary metallic taste when starting METFORMIN XR.

While METFORMIN XR rarely causes hypoglycemia (low blood sugar) on its own, it can occur if you do not eat enough, consume alcohol, or take other blood sugar-lowering medications.
